What the research actually says
We would rather point you at published work than at claims we made up ourselves. Here is what peer-reviewed research has found about pillows and neck pain — including where the evidence is still thin.
A systematic review of 11 studies (309 participants), graded with the GRADE method, found moderate evidence that a contoured pillow design, a height between 7 and 11 cm, and a cooling surface improve sleep quality and spinal alignment while decreasing sleep-related pain. A pillow height in that window produced the highest comfort ratings and the lowest cervical and cranial pressure.
Radwan et al., Journal of Bodywork and Movement Therapies, 2021A randomised controlled trial in Physical Therapy followed 64 people with chronic non-specific neck pain. Against an education-only control, changing the pillow produced a mean reduction of 8.7 points in neck pain, 8.4 in thoracic pain and 16.0 in headache. 69.3% of participants improved, against 39.4% in the control group.
Vanti C, Banchelli F, Marino C, Puccetti A, Guccione AA, Pillastrini P. Physical Therapy, 2019A systematic review and meta-analysis pooling 9 studies and 555 participants found a significant reduction in neck pain and in waking pain with an appropriate pillow, along with improved neck disability scores and higher satisfaction. The same analysis found no measurable effect on overall sleep quality — the benefit is on pain, not on how long you sleep.

Clinical Biomechanics, Vol. 85, 2021Being straight with you: an older review (Shields et al., New Zealand Journal of Physiotherapy, 2006) concluded there was insufficient evidence to recommend cervical pillows, and the trials it examined were small and of low quality.
